Requesting Medical Records (Patient Request)

Note: To request medical records from Durham Regional Hospital, call (919) 470-5172; to request records from Duke Health Raleigh Hospital, call (919) 954-3150.

In order to request a copy of your medical record, you will need to submit a written request to the Medical Information Release Unit (MIRU) of Duke University Hospital. You may use our Authorization to Protected Health Information form in English (or in Spanish version) In lieu of our form you can submit a letter to us authorizing release of medical records. This letter must include the following information:

1. Identifying information (your name, date of birth and Duke medical record number if known)
2. The name and address of the individual or institution you are authorizing to receive your records
3. The specific information you would like released and the associated time period (example: May 2001 to June 2002 and Cardiology Clinic Notes)
4. Signature of the patient or legal guardian and date you are initiating the request (Please note: if you are submitting a request on behalf of an adult for whom you are the executor or have power of attorney, you must submit a copy of the appropriate documentation)
5. An expiration date for the request. If no date is given, the authorization will be honored for one year.
6. A statement that the authorization may be revoked at any time, except to the extent that action has already been taken in reliance on the authorization.

If you authorize the release of medical records to a physician, hospital,  police department or school, there is no charge for the copying of records., For those requests for which a fee is assessed a prepayment letter will be generated for record copies. Payment must be remitted before records are sent.

Fee Schedule:
$10.00 for 1-14 pages   
$.75 per page for 15-25 pages
$.50 per page for 26-100 pages
$.25 per page for 101+ pages
